BtnCkOut Equipment Pool Loan Request
 The CMR Equipment Pool is available for faculty, staff and students for academic or official use.  Use this form to make a request.  Upon confirmation, items can be checked out from CMR (Miller 33) with a Colby ID.  Complete the last section of this form if you want CMR to deliver and set-up equipment for classroom use or event.
