The Garrison-Foster Health Center keeps medical records for seven years.  Alumni and current students may request copies of their records by submitting a completed medical records release form (see Request Form) or a written request, which includes full name, class year, date of birth and signature.  The request must state the specific information which needs to be released, and where the information needs to be faxed or mailed.  Records from non-Colby providers cannot be released.  The request can be faxed to (207) 859-4475 or mailed to:
