Student Health Forms
There are three forms that new students need to complete by July 15th: the Student Personal Health History Form, the Physician's Exam Form and the Immunization Form.
The Personal Health History Form can be completed on-line, once you have your Colby login and password. Your doctor will need to complete the Physician's Exam Form and the Immunization Form for you to submit. You may download the "Physician's Exam Form" and "Immunization Form" on this web page, or use the copies mailed to you. If you are unable to use the on-line form, you may download and fill out the pdf version of the form.
